 Important Note

Is an agent permitted to change any information on a policy?
A: Absolutely not. All changes to a policy must be made by the policyholder in writing.

If you do not have Adobe Acrobat Reader installed on your computer, you can download it FREE of charge from Adobe by pressing this button. Once you have Adobe Acrobat Reader on your Computer, you can select the form you wish to view or print from the list above.
When is the premium check cashed?
A: Premium checks are cashed immediately upon receipt at the Home Office.
When are agent’s renewal statements mailed?
A: Renewal statements are mailed on the 15th of each month.
Who are policies mailed to?
A: Policies are mailed directly to the insured. Provident American does not mail policies to agents for personal delivery.
How do I get assistance?
A: For specific questions regarding policy status, underwriting assistance, commission compensation, etc., you may contact your general agent or Provident’s marketing help desk at 1-800-924-3684.

For supply orders, contact your general agent.
When am I paid commission on my submitted business?
A: For All Products, an advanced commission of 6 months is paid upon issue of the product. Issue of these products usually occurs within 72 hours of receipt at the Home Office. The advance is paid on the Friday following issue of the policy.

Not all products are available in all states where we are licensed.  Some variations may occur between the same products in different states due to state laws and requirements.  All questions regarding policy terms should be directed to a licensed Provident American agent or to our Home Office at 1-800-924-3684.
