1.  We Must have Name, Street Address, City, State, Zip, and Agent Number for Delivery.

2.  For Express Service, please specify that your Supply Order be expressed delivered and specify method of special shipping or call 214-696-9091.

3.  Be Specific as to the Form Number, and the State when ordering.

4.  When mailing your Supply Order Form to the Agency Department for processing, make a copy for your records.

5.  All Orders are shipped by UPS Ground or First Class Mail.  Special service, such as Federal Express, UPS Next Day or Overnight, is at the Agent's expense.
